Every athlete must have a CLEARANCE CARD to participate in a sport at Cathedral Catholic High School.
CLEARANCE CARD information & physical MUST be updated every school year (Cathedral Catholic 2024-25 Physcial Form).

Below are the important steps every prospective student-athlete must complete to obtain a clearance card.

Important Note: ALL TRANSFER STUDENTS must also meet with the one of the Athletic Directors before they can be cleared to participate.
Please email athletics (if you haven’t been in contact already) to set up an appointment. This process can take some time so please don’t wait.

Athletic Office Hours 
Summer Hours from June 3 – August 13:  7am – 1pm
2024-25 School Year Hours:  7am – 3pm  
    Note: The office observes all school and federal holidays and will not be open on those days.
